What is a mole check?

Annual peace of mind, consultant-led.

A mole check is a focused dermatological examination of a mole or area of concern, combined with a wider visual scan of the surrounding skin. It uses dermatoscopy, a magnified, polarised view of the skin, which detects structural patterns invisible to the naked eye and significantly improves diagnostic accuracy compared with the naked-eye exam alone.

We recommend an annual mole check for adults with a personal or family history of skin cancer, more than 30 moles, fair skin, or significant cumulative sun exposure. For everyone else, a baseline check every 2–3 years is a sensible precaution.

Your 30 minutes, broken down

We allocate a full 30 minutes per appointment because rushed examinations miss things. The first few minutes are conversation, what you've noticed, your skin history, sun exposure, family history of melanoma or other skin cancers. This shapes how attentively we look at different areas and which moles you point out as concerning.

Next is a structured examination. Each lesion you've flagged is examined visually, then with a high-magnification polarised dermatoscope. The surrounding skin gets a careful visual scan, most patients have at least one mole they hadn't noticed that warrants a closer look. We talk you through what we see in plain language as we go.

The appointment closes with a discussion: findings, any recommendations (monitor, remove, follow up), and answers to whatever questions you have. A written report follows within 24 hours.

The five-letter framework, in plain English

Dermatologists assess moles using a simple mnemonic, ABCDE, that captures the five features most strongly associated with malignant change. **A is for Asymmetry**: imagine folding the mole in half through any axis. Benign moles are usually symmetrical; melanomas typically aren't. **B is for Border**: smooth, well-defined edges are reassuring; ragged, blurred or notched edges are not.

**C is for Colour**: a single shade of brown is normal; multiple shades within one mole, patches of black, red, blue, white or grey, is a red flag. **D is for Diameter**: larger than 6mm (the size of a pencil eraser) raises concern, though early melanomas can be smaller. **E is for Evolving**: any change over weeks or months matters more than any single feature on its own, growing, raising, changing colour, itching, bleeding or crusting.

No one ABCDE feature is diagnostic in isolation, and benign moles can score badly on one criterion. The skill is in the pattern, which is what dermatoscopy and a trained eye are for.

What you actually receive afterwards

Within 24 hours of your appointment we send a personalised written report by email. It includes: a summary of the lesions examined, the findings for each (benign / borderline / suspicious), photographs where they help illustrate what we saw, the recommendation for each lesion (no action, monitor, remove, refer), and an overall risk-stratification statement that influences how often we'd suggest you return.

With your consent, the report is also sent to your GP for their records, useful for continuity if a future GP wants context, and helpful if you ever need NHS dermatology referral down the line.

If anything is flagged for excision, we'll either book that in at the end of the appointment or arrange it to suit your timing. Most patients leave with a clear plan, not a list of unanswered questions.

Annual, every few years, or one-off

There's no single right answer, it depends on your baseline risk. Annual mole checks are recommended if you have any of: a first-degree relative with melanoma, a personal history of skin cancer, more than 30 moles, fair skin (Fitzpatrick types I or II), or significant cumulative sun exposure (childhood sunburn, frequent tropical holidays, sun-exposed occupation).

If none of those apply, a baseline check every 2–3 years is sensible, both as a precaution and to establish a documented record of what's normal for your skin. Some patients book one-off checks specifically because they've noticed a change in a mole; that's also entirely reasonable.

If you have more than ~30 moles, fair skin and a strong sun-exposure history, or any atypical naevi, total-body mole mapping is usually a better starting point than annual checks, the photographic record allows side-by-side comparison that catches subtle changes a fresh eye can miss.
